【摘要】目的:探讨普外科围术期疼痛伴有抑郁情绪的睡眠障碍患者匹兹堡睡眠质量指数(PSQI)评分的变化及与患者发生认知功能损害的关系。方法:选取我院2016年10月至2019年10月期间普外科90例围术期疼痛伴有抑郁情绪的睡眠障碍患者(抑郁组),选择同期90例普外科围术期单纯睡眠障碍患者(对照组);对比两组的PSQI各项指标评分、认知功能评分,并采用多元线性回归方法分析抑郁组患者认知功能评分与PSQI及抑郁程度的相关性。结果:抑郁组和对照组的睡眠质量、睡眠效率、催眠药物评分比较,差异无统计学意义(P>0.05);抑郁组的入睡时间、睡眠时间、睡眠障碍、日间功能、PSQI总分均高于对照组(P<0.05);抑郁组的即刻记忆、视觉广度、延时记忆及RBANS总分均低于对照组(P<0.05);多元线性回归模型结果显示HAMD评分、PSQI评分与患者的RBANS总分呈负相关(P<0.05),年龄、受教育年限与患者的RBANS总分呈正相关(P<0.05) 。结论:普外科围术期疼痛合并抑郁情绪的睡眠障碍患者睡眠障碍病情更加严重较单纯的睡眠障碍患者,睡眠障碍与抑郁情绪二者相互作用对患者的认知功能有一定程度的不良影响。

[Abstract] objective: to investigate the changes of Pittsburgh sleep quality index (PSQI) score and its relationship with cognitive impairment in patients with perioperative pain and depression in general surgery. Methods: from October 2016 to October 2019, 90 patients with perioperative pain accompanied by depression in general surgery department were selected (depression group). Meanwhile, 90 patients with perioperative simple sleep disorder in general surgery department were selected (control group). The PSQI scores and cognitive function scores of the two groups were compared, and the correlation between cognitive function scores and PSQI and depression degree of the depressed group was analyzed by using multiple linear regression method. Results: there was no significant difference in sleep quality, sleep efficiency and hypnotic score between the depression group and the control group (P>0.05). The sleep time, sleep time, sleep disturbance, daytime function and total PSQI scores of the depression group were all higher than those of the control group (P<0.05). Immediate memory, visual span, delayed memory and RBANS in the depression group were all lower than those in the control group (P<0.05). The results of multiple linear regression model showed that HAMD score and PSQI score were negatively correlated with the total RBANS of patients (P<0.05), and age and years of education were positively correlated with the total RBANS of patients (P<0.05). Conclusion : the perioperative pain and depression in general surgery patients with sleep disorders are more serious than those with simple sleep disorders.
