【摘要】目的 研究齐拉西酮对老年脑器质性精神障碍患者内环境及疗效的影响。方法 纳入我院从2017年2月~2019年2月收治的90例老年脑器质性精神障碍患者进行研究。将其按照随机数表法等分成观察组和对照组。对照组采用奥氮平治疗,观察组则予以齐拉西酮治疗。比较两组临床疗效,治疗前后糖脂代谢指标水平[主要涵盖空腹血糖、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]以及瘦素、催乳素水平,不良反应发生情况。结果 观察组治疗总有效率和对照组相比不明显(均P>0.05)。治疗后观察组空腹血糖、TG、TC、LDL-C、HDL-C水平均显著低于对照组(均P<0.05)。治疗后观察组瘦素水平显著低于对照组,而催乳素水平显著高于对照组(均P<0.05)。观察组及对照组各项不良反应发生率对比均不明显(均P>0.05)。结论 齐拉西酮治疗老年脑器质性精神障碍患者的疗效和奥氮平相当,但在改善患者内环境方面的效果明显更优,且有利于改善瘦素、催乳素水平,安全性较好,值得临床推广应用。

[Abstract] Objective To study the effects of ziprasidone on internal environment and efficacy in elderly patients with organic mental disorders of the brain. Methods A total of 90 elderly patients with organic brain disorders admitted to our hospital from February 2017 to February 2019 were included in the study. They were divided into observation group and control group according to the random number table method. The control group was treated with olanzapine, while the observation group was treated with ziprasidone. The clinical efficacy of the two groups was compared, and the glucose and lipid metabolism indexes before and after treatment [mainly including fasting blood glucose, triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (ldl-c), high-density lipoprotein cholesterol (hdl-c)], leptin and prolactin levels, and the occurrence of adverse reactions. Results The total effective rate of treatment in the observation group was not significant compared with that in the control group (all P >0.05). After treatment, fasting glucose, TG, TC, ldl-c and hdl-c levels in the observation group were significantly lower than those in the control group (all P < 0.05). After treatment, leptin level in the observation group was significantly lower than that in the control group, while prolactin level was significantly higher than that in the control group (all P < 0.05). The comparison of the incidence of adverse reactions between the observation group and the control group was not significant (all P >0.05). Conclusion Ziprasidone is equivalent to olanzapine in the treatment of senile patients with organic mental disorders, but has better effect in improving the internal environment of the patients, and is beneficial to improve leptin and prolactin levels.
