丁苯酞软胶囊联合高压氧治疗脑卒中后血管性痴呆对患者TGF-β、β-Ap水平的影响

【摘要】
目的 观察丁苯酞软胶囊联合高压氧治疗脑卒中后血管性痴呆对患者转化生长因子-β(TGF-β)、β淀粉样蛋白(β-Ap)水平的影响,为临床治疗提供参考。

方法 选取我院(2017年5月~2020年2月)收治的脑卒中后血管性痴呆患者130例,根据治疗

方法分为2组,每组65例。2组患者均给予降血压、调血脂、抗血小板等常规对症治疗,对照组给予高压氧治疗,观察组在对照组基础上联合丁苯酞软胶囊治疗。记录2组治疗前后脑血流灌注(CBF)、TGF-β、β-Ap、神经生长因子(NGF)、天冬氨酸特异性半胱氨酸蛋白酶-3(Caspase-3)及脑电图频谱功率指标的变化,统计2组疗效。

结果 观察组总有效率(90.77%)高于对照组(76.92%),差异显著(P<0.05)。治疗后2组TGF-β、NGF逐渐上升(P<0.05),β-Ap、Caspase-3逐渐下降(P<0.05),观察组治疗后TGF-β、NGF明显较对照组高(P<0.05),β-Ap、Caspase-3明显较对照组低(P<0.05)。治疗后2组CBF逐渐上升(P<0.05),进一步组间比较发现,观察组CBF高于对照组(P<0.05)。治疗后2组脑电图频谱功率中α频谱逐渐上升(P<0.05),θ频谱、δ频谱逐渐下降(P<0.05),观察组脑电图频谱功率优于对照组(P<0.05)。

结论 丁苯酞软胶囊联合高压氧治疗脑卒中后血管性痴呆可改善患者脑血流灌注和脑电图频谱功率,调节相关因子的表达,提高疗效。

ABSTRACT

【Abstract】
Objective To observe the effect of butylphthalide soft capsule combined with hyperbaric oxygen in the treatment of post-stroke vascular dementia on the levels of transforming growth factor-β (TGF-β) and β-amyloid protein (β-Ap), and provide clinical treatment reference.

Methods A total of 130 patients with post-stroke vascular dementia admitted to our hospital (from May 2017 to February 2020) were selected and divided into 2 groups according to the treatment method, with 65 cases in each group. Patients in both groups were given conventional symptomatic treatments such as lowering blood pressure, regulating blood lipids, and anti-platelet. The control group was given hyperbaric oxygen therapy, and the observation group was treated with butylphthalide soft capsules on the basis of the control group. Record the cerebral blood perfusion (CBF), TGF-β, β-Ap, nerve growth factor (NGF), aspartate-specific caspase-3 (Caspase-3) and electroencephalogram before and after treatment in the two groups The changes of the spectrum power index are used to calculate the efficacy of the 2 groups.

Results The total effective rate of the observation group (90.77%) was higher than that of the control group (76.92%), and the difference was significant (P<0.05). After treatment, TGF-β and NGF in the two groups gradually increased (P<0.05), and β-Ap and Caspase-3 gradually decreased (P<0.05). After treatment, TGF-β and NGF in the observation group were significantly higher than those in the control group (P<0.05) ), β-Ap and Caspase-3 were significantly lower than those in the control group (P<0.05). After treatment, the CBF of the two groups increased gradually (P<0.05), and further comparison between the groups found that the CBF of the observation group was higher than that of the control group (P<0.05). After treatment, the alpha spectrum of the EEG spectrum power in the two groups gradually increased (P<0.05), and the θ spectrum and δ spectrum gradually decreased (P<0.05). The EEG spectrum power of the observation group was better than that of the control group (P<0.05).

Conclusion   Butylphthalide soft capsule combined with hyperbaric oxygen in the treatment of post-stroke vascular dementia can improve the cerebral blood perfusion and EEG spectrum power in patients, regulate the expression of related factors, and improve the curative effect.
