[Abstract]Objective: To compare the effect of transthoracic contrast echocardiography (TTE) with transesophageal contrast echocardiography (TEE) for detecting right to left shunt (RLS) combined with patent foramen ovale (PFO) in patients with unexplained stroke and hemicrania.MRTHODS: During April,2015 to April,2016，28 cases of patients with unexplained stroke and 22 cases of patients with hemicrania were selected，All patients were followed by TTE and TEE. The TTE imaging frequency was 2-4MHz, and the TEE imaging frequency was 7MHz. The detection rate of PFO-RLS, semi-quantitative shunting grade and postoperative complication rate.RESULTS:TThe detection rate of PFO-RLS and semi-quantitative shunting grade of TFO were significantly higher than those of TEE (P<0.05). The incidence of postoperative complications of TTE was 0, which was significantly lower than that of TEE 11.9% (P <0.05). In addition, the specificity, sensitivity, and accuracy of TTE for the diagnosis of PFO-RLS was also significantly better than that of TEE (P <0.05)..CONCLUSION: The rate of PFO-RLS in patients with unexplained stroke or migraine is high, the accuracy rate is high, the complication rate is low, and semi-quantitative shunt is more accurate. It is worthy of popularization and application.
