急性脑梗死患者IGF-1、ET-1、Hcy水平与认知功能障碍的相关性分析

摘要

【摘 要】
目的:探讨急性脑梗死患者胰岛素生长因子-1(IGF-1)、内皮素-1(ET-1)、同型半胱氨酸(Hcy)水平与认知功能障碍的相关性。

方法:选取2013年2月至2016年2月我院收治的100例急性脑梗死患者作为研究对象,按照简易精神状态量表(MMSE)评分将患者分为认知功能障碍组(57例)和无认知功能障碍组(43例),并进一步将认知功能障碍组患者按照MMSE评分分为轻度认知障碍组(24例)、中度认知障碍组(25例)和重度认知障碍组(8例)。比较各组患者IGF-1、ET-1、Hcy水平差异,及其与MMSE评分的相关性。

结果:认知功能障碍组患者IGF-1水平明显低于无认知功能障碍组(P<0.05),ET-1、Hcy水平明显高于无认知功能障碍组(P<0.05);随着认知功能障碍程度的加重,患者IGF-1水平随之降低,ET-1、Hcy水平随之升高,且不同认知功能障碍程度患者间IGF-1、ET-1、Hcy水平存在显著差异(P<0.05);急性脑梗死患者IGF-1水平与MMSE评分呈负相关(r=-0.561,P=0.006),ET-1水平与MMSE评分呈负相关(r=-0.327,P=0.021),Hcy水平与MMSE评分呈负相关(r= -0.117,P= 0.397)。

结论:急性脑梗死患者IGF-1、ET-1、Hcy水平与认知功能障碍密切相关,可以作为判断急性脑梗死后认知功能障碍严重程度的的生物学检测指标。

ABSTRACT

[Abstract]
Objective: To investigate the correlation between IGF-1, ET-1, Hcy and cognitive dysfunction in patients with acute cerebral infarction.

Methods: 100 cases of acute cerebral infarction were selected from February 2013 to February 2016 in our hospital as the research object, according to the MMSE scores of the patients were divided into cognitive dysfunction group (57 cases) and no cognitive dysfunction group (43 cases), and the patients with cognitive impairment in MMSE were divided into mild cognitive impairment group (24 cases), the degree of cognitive impairment group (25 cases) and severe cognitive impairment group (8 cases). Compared the levels of IGF-1, ET-1 and Hcy, and their correlation with MMSE score in each group.

Results: The level of IGF-1 in patients with cognitive dysfunction was significantly lower than that in non cognitive impairment group (P<0.05), and the levels of ET-1 and Hcy were significantly higher than those without cognitive impairment group (P<0.05); With the increase of the degree of cognitive impairment, the level of IGF-1 in patients with decreased ET-1 and Hcy levels increased, and there is obvious difference among different degree of cognitive impairment between patients with IGF-1, ET-1, Hcy (P<0.05); The level of IGF-1 in patients with acute cerebral infarction was negatively correlated with MMSE score (r=-0.561, P=0.006), and ET-1 level was negatively correlated with MMSE score (r=-0.327, P=0.021), and Hcy level was negatively correlated with MMSE score (r=-0.117, P=0.397).

Conclusion  : The level of IGF-1, ET-1 and Hcy in patients with acute cerebral infarction is closely related to cognitive dysfunction. It can be used as a biological indicator to determine the severity of cognitive impairment after acute cerebral infarction.
