阿立哌唑联合奥氮平对老年精神分裂症患者糖脂代谢及心肌酶的影响

目的:探讨阿立哌唑联合奥氮平对老年精神分裂症患者糖脂代谢及心肌酶的影响,分析联合治疗可能作用机制。

方法:将60例老年精神分裂症患者随机分为观察组和对照组各30例,对照组给予奥氮平治疗,观察组给予阿立哌唑联合奥氮平治疗,治疗12周后,比较两组患者糖脂代谢、心肌酶、临床疗效、不良反应等指标。

结果:观察组有效率93.33%明显高于对照组73.33%(P<0.05);空腹血糖(CLU)、总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白(LDL)均明显低于对照组,高密度脂蛋白(HDL)明显高于对照组(P<0.05);血清谷草转氨酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)均明显低于对照组(P<0.05, P<0.01);流涎等不良反应13.33%明显低于对照组36.67%(P<0.05)。

结论:阿立哌唑联合奥氮平有助于提高老年精神分裂症患者临床疗效,降低不良反应,且对糖脂代谢、心肌酶等影响较小。

ABSTRACT

Abstract:
Objective: To investigate the effect of aripiprazole in combination with olanzapine on glucolipid metabolism and myocardial enzyme of patients with elderly schizophrenia and to analyze the possible mechanism of combined treatment.

Methods: A total of 60 cases of patients with elderly schizophrenia were randomly divided into Observation Group and Control Group, with 30 cases in each group. Patients in Control Group were treated by olanzapine, and those in Observation Group were treated by aripiprazole in combination with olanzapine. After 12 weeks of treatment, glucolipid metabolism, myocardial enzymes, clinical efficacy, adverse reactions and other indicators of the two groups were compared.

Results: The effective rate of Observation Group was 93.33% and significantly higher than 73.33% of Control Group (P<0.05); the levels of GLU, total cholesterol(TC), triglyceride(TG) and low density lipoprotein(LDL) were significantly lower than in Control Group; the level of high-density lipoprotein(HDL) was significantly higher than in Control Group(P< 0.05); serum aspartate amino transferase (AST), lactate dehydrogenase (LDH), creatine kinase (CK) and creatine kinase-MB (CK-MB) were significantly lower than in Control Group(P<0.05, P<0.01); the incidence rate of salivation and other adverse reactions was 13.33% and significantly lower than 36.67% of Control Group (P<0.05).

Conclusion  : Aripiprazole in combination with olanzapine is conductive to improving the clinical efficacy of patients with elderly schizophrenia and reducing adverse reactions, besides, the therapy exerts little effect on glucolipid metabolism and myocardial enzyme.
