乳腺癌患者术后性心理状态及影响生存质量的相关因素分析

摘要


目的探讨分析乳腺癌患者术后性心理状态及影响生存质量的相关因素。

方法以2014年1月至2016年1月我院收治的79例乳腺癌患者为研究对象。另选取同期医院收治的60例乳腺良性肿瘤患者作为对照组。采用焦虑自评量表(SAS)及抑郁自评量表(SDS)来评价并比较两组患者的焦虑和抑郁水平,采用乳腺癌患者生命质量测定量表(FACT-B)对两组患者的生存质量进行分析测评,此外对不同文化程度与年龄段患者的心理状态与生存质量状况进行比较。

结果观察组SDS、SAS评分分别为(54.41±6.46)、(50.03±5.53),对照组两项评分分别为(32.39±4.82)、(39.32±3.36),组间比较观察组高于对照组,差异具有统计学意义(P<0.05)。观察组的生理状况、社会/家庭状况、情感状况、功能状况及附加关注各维度的评分均明显低于对照组,差异具有显著性(P<0.05)。不同年龄及文化程度患者的SDS、SAS及FACT-B总分存在显著性差异(P<0.05);组间多重比较

结果显示随年龄的增加SDS、SAS评分呈逐渐降低趋势,FACT-B总分呈上升特点,而文化程度增加则伴随SDS、SAS评分的上升与FACT-B总分的下降。

结论乳腺癌患者术后多存在严重的焦虑与抑郁情绪,生存质量明显下降,文化程度、年龄甚至婚姻状况都可视为影响生存质量的重要因素。

ABSTRACT


Objective: To discuss the postoperative psychological status of patients with breast cancer and factors affecting survival quality.

Methods: 79 cases of patients with breast cancer in our hospital from January 2014 to January 2016 were selected as the research object.60 cases of patients with benign breast tumor in our hospital in the same period were selected as the control group.SAS and SDS were used to evaluate the anxiety and depression levels of both groups. FACT-B was used to evaluate the survival quality of both groups. Besides, the psychological status and survival quality of patients with different education background and age were compared.

Results: The SDS and SAS score of the observation group were (54.41±6.46) and (50.03±5.53), while (32.39±4.82) and (39.32±3.36) for the control group. The observation group was higher than the control group, with statistically significant difference (P<0.05).The physiological status, social/family status, emotional status, and function condition, and additional focus of the observation group were lower than the control group. The difference was significant, with P<0.05.There were significant difference of SDS, SAS, and FACT-B scores of among patients of different age and education background, with P<0.05.With the increase of age, SDS and SAS scores decreased, while FACT-B score increased. With the increase of education background, SDS and SAS scores increased, while FACT-B score decreased.

Conclusion  : Postoperative patients with breast cancer often have severe anxiety and depression, with survival quality decreasing significantly. Education background, age, and even marital status can be regarded as vital factors affecting survival quality.
