摘要:目的:分析并探讨不同剂量多奈哌齐联合奥氮平治疗阿尔茨海默病的临床效果,以及对认知功能和精神症状的改善情况,从而为临床治疗方案选择提供依据。方法:选取2014年10月至2016年10月在医院接受治疗的阿尔茨海默病患者120例为研究对象,随机分为观察组A与观察组B,每组60例。两组均给予奥氮平治疗,在此基础上,观察组A给予大剂量多奈哌齐,观察组B给予小剂量多奈哌齐。结果:观察组A治疗有效率为81.67%,观察组B治疗有效率为63.33%。观察组A治疗有效率明显高于观察组B(P<0.05)。观察组A治疗后NPI总分为(23.25±4.25)分,观察组B为(26.35±4.23)分。两组治疗后NPI总分均有显著改善,观察组A改善更加明显(P<0.05)。观察组A治疗后MMSE总分为(17.46±1.95)分,观察组B为(15.54±2.04)分。观察组A治疗后NPI苦恼总分为(33.64±4.16)分,观察组B为(37.53±4.64)分。两组治疗后NPI苦恼总分均有显著改善,观察组A改善更加明显(P<0.05)。两组治疗后MMSE总分均有显著改善,观察组A改善更加明显(P<0.05)。两组不良反应发生率无显著差异(P>0.05)。结论:大剂量多奈哌齐联合奥氮平治疗阿尔茨海默病的临床效果显著,值得推广。

Abstract: Objective: To analyze and explore the clinical effect of different doses of donepezil combined with olanzapine in the treatment of Alzheimer's disease, and to improve the cognitive function and psychiatric symptoms, so as to provide the basis for clinical treatment options. Methods: 120 patients with Alzheimer's disease who were treated in hospital from October 2014 to October 2016 were randomly divided into observation group A and observation group B, 60 patients in each group. Both groups were treated with olanzapine. On this basis, observation group A was given high doses of donepezil, and observation group B was given low dose donepezil. Results: The effective rate of treatment group A was 81.67%, and the effective rate of treatment group B was 63.33%. The effective rate of treatment group A was significantly higher than that of observation group B (P <0.05). The total score of NPI was (23.25 ± 4.25) and observed in group B (26.35 ± 4.23). The total score of NPI was significantly improved after treatment, and the improvement of group A was more obvious (P <0.05). The total score of MMSE was (17.46 ± 1.95) in observation group A and 15.54 ± 2.04 in observation group. Observation group A after treatment NPI distress total score (33.64 ± 4.16) points, observation group B was (37.53 ± 4.64) points. After treatment, the scores of NPI were significantly improved, and the improvement of group A was more obvious (P <0.05). The scores of MMSE were significantly improved after treatment, and the improvement of group A was more obvious (P <0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P> 0.05). Conclusion : The clinical effect of high dose donepezil combined with olanzapine in the treatment of Alzheimer's disease is remarkable and worthy of promotion.
