引导式教育训练用于老年脑卒中后偏瘫的康复效果及相关因素的初步研究

目的 探讨引导式教育训练对老年脑卒中后偏瘫患者恢复效果的影响,并分析患者康复效果的相关因素。

方法 将2014年1月至2016年12月我院收治的86例老年脑卒中后偏瘫患者随机均分为2组,各43例。2组均接受神经科常规干预,观察组同时接受引导式教育训练。分别于干预前、干预3月后测评2组Fugl-Meyer运动功能评分(FMA)、Barthel指数(BI)。以干预后BI为因变量,患者治疗方案、一般资料及临床病理特征为自变量,采用逐步多元回归分析明确患者康复效果的相关因素。

结果 干预前2组FMA上肢及下肢部分、BI得分对比,差异无统计学意义(P>0.05);干预后,观察组上述得分均明显高于对照组,差异有统计学意义(P<0.05)。干预后患者BI的相关因素包括:干预前神经功能缺损程度、发病至干预时间、干预方案,回归方程调整后的判定系数为0.978。

结论 引导式教育训练能够提升老年脑卒中后偏瘫的康复效果。患者干预后BI的相关因素除与干预方案有关外,还与干预前神经功能缺损程度、发病至干预时间有关。

ABSTRACT

Abstract:
Objective To investigate the effect of conductive education and training on the recovery of elderly hemiplegia patients after stroke, and to analyze the related factors of the rehabilitation effect.

Methods 86 elderly patients with hemiplegia after stroke treated in our hospital from January 2014 to December 2016 were randomly divided into 2 groups, with 43 cases in each group. Both groups received routine neurologic intervention, and the observation group received conductive education and training at the same time. The scores of Fugl-Meyer assessment scale (FMA) and Barthel index (BI) before and 3 months after intervention in 2 groups were observed and compared. BI after treatment was taken as the dependent variable, patients' treatment plan, general data and clinicopathological characteristics were taken as the independent variables, then a stepwise multiple regression analysis was used to identify related factors of rehabilitation effect.

Results Before intervention, there were no significant differences in the scores of upper and lower extremities parts of FMA and in BI scores between the 2 groups (P > 0.05). After intervention, the above scores in the observation group were significantly higher than those in the control group, with a statistically significant difference (P < 0.05). The related factors of BI after intervention included: the degree of neurological deficit before intervention, the time from onset to intervention and intervention plan, and the coefficient of determination after adjustment of regression equation was 0.978.

Conclusion   Conductive education and training can improve the rehabilitation effect of elderly patients with hemiplegia after stroke. The related factors of BI after intervention included the degree of neurological deficit before intervention, the time from onset to intervention and intervention plan.
