呼出气一氧化氮测定对于变应性鼻炎患者下气道异常的临床诊断研究

【摘要】
目的:探究FeNO检查作为变应性鼻炎发展为哮喘的炎症指标的可行性。

方法:选取2017年1月一2017年10月期间因变应性鼻炎(AR)来我院耳鼻咽喉科门诊入院就诊的212例AR患者作为研究对象,同期选取40名健康志愿者列为对照组。对纳入研究的所有受试者分别进行上、下气道NO含量检测及肺通气功能检查。对检测数据进行统计分析及相关性检测。

结果:上气道NO含量中,中重度AR患者FENO含量显著高于其他两组患者(P<0.05),而对照组及轻度AR患者上气道NO含量无差异(P>0.05)。下气道NO含量中,中重度AR患者下气道NO含量显著高于其他两组患者(P<0.05),且轻度AR患者上气道NO含量亦显著高于对照组患者(P<0.05)。3组患者上、下气道NO含量不存在显著相关性(P>0.05)。

结论:FENO检测对于变应性鼻炎合并下气道炎症反应的诊断存在可行性,可以作为变应性鼻炎发展为哮喘的炎症指标,指导临床治疗,但上气道NO检测的临床应用意义有待进一步研究评价。

ABSTRACT


Objective: to explore the feasibility of FeNO examination as an inflammatory marker for the development of allergic rhinitis into asthma.

Methods: from October 1st 2015 to October 2017, allergic rhinitis was selected. AR) 212 AR patients who were admitted to our department of otolaryngology were studied. In the same period, 40 healthy volunteers were selected as control group. No content in upper and lower airway and pulmonary ventilation function were measured in all subjects.

Results : The content of NO in the upper airway was significantly higher than that in the other two groups ( P < 0.05 ) . The NO content in the lower airways was significantly higher than that in the other two groups ( P < 0.05 ) . The diagnosis of lower airway inflammatory reaction is feasible, which can be used as an inflammatory index for allergic rhinitis to develop into asthma and guide clinical treatment. However, the clinical application of no detection in upper airway needs further study and evaluation.
