【摘要】： 目的：探讨长期住院慢性精神分裂症患者血清甲状腺激素以及性激素水平及其影响因素。方法：收集在我院长期住院的单一用药者，氯丙嗪组51例，氯氮平组67例，作为研究组，健康对照组60例，于同一时间检测甲状腺激素（T3, T4, FT3, FT4）以及性激素水平（PRL, E2, P, T），并分析研究组激素水平与病程、服药时长、症状等的相关性。结果：氯丙嗪组与氯氮平组的FT4、T3、PRL均明显高于对照组（p< 0.01），E2明显低于对照组（p< 0.01），氯丙嗪组的PRL明显高于氯氮平组（p< 0.01），FT3、T4、TSH、T、P在两组与对照组之间，以及两组之间均无统计学差异。FT3与PANSS总分、阴性量表分、一般病理分负相关（r=-0.237， r=-0.223, r=-0.204,均p＜0.05）。FT4与年龄呈现正相关（r=0.228，p=0.013），T3与阴性量表分负相关（r=-0.184，p=0.046）。结论：长期住院经单一用药治疗的精神分裂症患者（氯丙嗪、氯氮平）的甲状腺功能紊乱，PRL水平明显升高，氯丙嗪对PRL的影响高于氯氮平，低E2水平可能是精神分裂症的属性标志。
Abstract： Objective: To explore the serum thyroid hormone and sex hormone levels in patients with chronic schizophrenia, as well as its influencing factors. Methods：Long-term antipsychotic drug mono-therapy hospitalization patients in our hospital were collected. The number of patients with chlorpromazine mono-therapy was 51, clozapine mono-therapy was 67, and the healthy control group was 60.Thyroid hormones (T3, T4, FT3, FT4) and sex hormone (PRL, E2, P, T) levels in those patients were detected, and the course of the disease, medication treatment duration, and symptoms were also analyzed. Results: The FT4, T3, PRL level of chlorpromazine and clozapine group were significantly higher than those of the control group’s (p<0.01), E2 was significantly lower than the control group (p < 0.01).The PRL level of chlorpromazine group was obviously higher than that of clozapine group’s (p < 0.01).No statistical difference was found between the two groups or between the control group, which including FT3, T4, TSH, T or P.FT3 was negatively correlated with the PANSS score, negative scale and general pathology (r=-0.237, r=-0.223, r=-0.204, all p<0.05).FT4 was positively correlated with the age (r=0.228,p=0.013), T3 and negative scale were negatively correlated (r=-0.184,p=0.046).Conclusion : The thyroid in long-term antipsychotic drug mono-therapy (chlorpromazine, clozapine)hospitalization patients with schizophrenia were dysfunction,the PRL level was obviously elevated, especially in chlorpromazine treated patients. The low level of E2 may be attributed symbol of schizophrenia.
