呼出气一氧化氮和过敏原检测在1~6岁儿童喘息性疾病中的应用

摘要

【摘要】
目的 探讨呼出气一氧化氮(fractional exhaled nitric oxide,FENO)和过敏原检测在1~6岁儿童喘息性疾病中的应用价值。

方法 回顾性分析我院自2017年1月至2018年1月治疗的257例1~6岁呼吸道疾病患儿的临床资料,根据是否出现喘息症状分为喘息组(163例)和非喘息组(94例),均进行FENO和过敏原检测,定量检测血清总免疫球蛋白E(tIgE)和特异性免疫球蛋白(sIgE);比较两组治疗前FENO水平、食入过敏原及吸入过敏原阳性率、血清tIgE及sIgE水平。

结果 喘息组治疗前FENO水平为(85.84±36.05)bbp,显著高于非喘息组(17.84±8.05)bbp,差异有统计学意义(P<0.01);两组治疗前食入过敏原阳性率、吸入过敏原阳性率比较,差异均有统计学意义(P<0.05);喘息组治疗前血清tIgE及尘螨、蛋清、牛奶sIgE水平均显著高于非喘息组,差异均有统计学意义(P<0.05);喘息组治疗后FENO、血清tIgE及尘螨、蛋清、牛奶sIgE水平均较治疗前显著降低,差异有统计学意义(P<0.05);经Pearson相关性分析,FENO水平与血清tIgE及尘螨、蛋清、牛奶sIgE水平均呈正相关(P<0.05)。

结论 FENO和过敏原检测有助于协同提高1~6岁儿童喘息性疾病的管理和评估水平,值得进一步研究应用。

ABSTRACT

【Abstract】
Objective To investigate the value of fractional exhaled nitric oxide (FENO) and allergen detection in asthmatic children aged 1 to 6 years.

Methods The clinical data of 257 children with respiratory disease from 1 to 6 years old who were treated in our hospital from January 2017 to January 2018 were retrospectively analyzed. The patients were divided into a wheezing group (163 cases) and a non-wheezing group according to whether wheezing symptoms were present ( In 94 patients), all FENO and allergens were detected and the serum total immunoglobulin E (tIgE) and specific immunoglobulin (sIgE) were quantified; FENO levels, ingested allergens, and inhaled allergens were positive in both groups before treatment. Rates, serum tIgE, and sIgE levels.

Results The FENO level before treatment in the wheez group was (85.84±36.05)bbp,which was significantly higher than that in the non-wheezing group (17.84±8.05)bbp,and the difference was statistically significant (P<0.01);The positive rate of ingested allergens and the positive rate of inhaled allergens before treatment in both groups were statistically significant(P<0.05);The levels of serum tIgE, dust mite, egg white and milk sIgE in the wheez group were significantly higher than those in the non-wheezing group(P<0.05);The levels of FENO, serum tIgE, dust mite, egg white and milk sIgE in the wheezing group were significantly lower than those before treatment, and the difference was statistically significant(P<0.05);By Pearson correlation analysis,FENO levels were positively correlated with serum tIgE and dust mite, egg white, and milk sIgE levels(P<0.05).

Conclusion   FENO and allergen detection help synergistically improve the management and assessment of asthmatic illness in children aged 1 to 6 years. It is worth further research and application.
