【摘要】 目的:探究高压氧联合司来吉兰对帕金森睡眠障碍患者的睡眠质量的影响及其对血浆GABA和血清ADPN的影响。方法:选择2014年1月-2016年1月来我院进行治疗的PD睡眠障碍患者128例作为研究对象,按照随机数字表法分为研究组64例和对照组64例。对照组给予高压氧进行治疗,研究组给予高压氧联合司来吉兰进行治疗。观察患者睡眠质量、睡眠相关指标、血浆γ-氨基丁酸(GABA)和血清脂联素(ADPN)水平,以及不良反应状况。结果:治疗后,两组患者的PSQI、ESS、UPDRS-III及H-Y分级均显著性下降(P<0.05),且研究组患者的改善程度显著优于对照组(P<0.05),差异均具有统计学意义;研究组患者的睡眠效率和睡眠时间显著增加,日间嗜睡程度减少,增加SWS及REM 期时间,Ⅰ期睡眠比例减少、REM 期和Ⅱ期睡眠比例增加;治疗后,两组患者的GABA和APDN水平均显著升高(P<0.05),且研究组水平显著优于对照组(P<0.05);研究组患者的不良反应率为18.75%,对照组患者的不良反应率为15.63%,两组患者相比无显著性差异(P>0.05)。结论:高压氧联合司来吉兰能够改善帕金森患者睡眠障碍,通过增加血浆GABA和ADPN水平,有效缓解患者的神经损伤,降低炎症反应,达到治疗帕金森的目的。

[Abstract] Objective:To explore the effect of the hyperbaric oxygenation combined with selegiline on the sleep quality of patients with Parkinson's disease and the levels of plasma GABA and serum ADPN. Method: Selected 128 patients with Parkinson's disease sleep disorder who had been treated in our hospital from Jan 2014 to Jan 2016, and equally divided them into the control group and the study group. The study group was treated with hyperbaric oxygenation combined with selegiline, while the control group was treated with hyperbaric oxygenation. To observe the quality of sleep, related indicators of sleep, the levels of GABA and serum adiponectin (ADPN) and the adverse reactions. Results: After treatment, PSQI, ESS, UPDRS-III and H-Y were significantly decreased in both groups (P<0.05), and the improvement degree in the study group were significantly better than that in the control group(P<0.05); The sleeping efficiency and sleeping time significantly were increased, daytime sleepiness degree reducing, increase SWS and REM period time, stageⅠsleep ratio decrease, REM andⅡsleep ratio were increased; After the treatment, the levels of GABA and APDN in both groups were increased(P<0.05), and the levels in the study was higher than that in the control group(P<0.05); Besides, the adverse reaction in the two groups has no significant difference(P>0.05). Conclusion : The hyperbaric oxygenation combined with selegiline can improve the Parkinson's disease sleep disorder, by improving the levels of GABA and ADPNA, effectively relieve the nerve injury, reduce the inflammatory response, and achieve the goal of treating Parkinson's disease.
