目的:探讨褪黑素对自闭症儿童睡眠障碍的影响。 方法:计算机检索CNKI、WanFang data、VIP、CBM、Embase、PubMed、The Cochrane library,中文检索词选择“自闭症/孤独症”、“褪黑素”、“睡眠障碍/失眠/不寐/睡眠不足”,英文检索词选择“Autism/autism spectrum disorder”、“ Melatonin”、“dyssomnias/insomnia/sleep deprivation”。检索时间均为建库至2018年10月31日,纳入随机对照试验(RCT)研究,对纳入的文献进行质量评价,采用RevMan5.3进行meta分析,并对结果进行亚组分析和敏感性分析。结果:共纳入文献8篇,397名患者。Meta分析的结果显示,褪黑素治疗自闭症儿童睡眠障碍在缩短睡眠潜伏期[SMD(95%CI)=-1.34(-1.61,-1.07),P<0.00001]、减少夜间醒来次数[SMD(95%CI)=-0.37(-0.60,-0.13),P=0.003]、延长总睡眠时间[SMD(95%CI)=1.19(0.64,1.75),P<0.00001]方面明显优于对照组。结论:褪黑素对于缓解自闭症儿童睡眠障碍有一定疗效,对于缩短睡眠潜伏期、减少夜间醒来次数、延长总睡眠时间有一定的意义。受到纳入文献数量及质量的限制,本研究的结论仍需更多高质量研究进行验证。

Retrieval time are built libraries to October 31, 2018, into a randomized controlled trial (RCT) study, the quality evaluation of the literature of the included, adopt RevMan5.3 meta analysis, and the results are subgroup analysis and sensitivity analysis. Results: a total of 8 into literature, 397 patients. Meta analysis results show that melatonin treatment for autistic children sleep disorders in shorten the incubation period of sleep [SMD (95% CI) = 1.34 (1.61, 1.07), P < 0.00001), reducing frequency of nighttime awakenings [SMD (95% CI) = 0.37 (0.60, 0.13), P = 0.003), extend the total sleep time [SMD (95% CI) = 1.19 (0.64, 1.75), P < 0.00001] is better than that in control group. Conclusion : melatonin for alleviating the autistic children sleep disorders have certain curative effect, to shorten the incubation period of sleep, reducing frequency of waking up at night, to extend the total sleep time has a certain significance. Limited in the number and quality of literature, the conclusions of this study still need more high quality research for validation.
