帕金森病患者抑郁症状与睡眠障碍及认知功能的相关性分析

摘要

【摘要】
目的 分析帕金森病(Parkinson's disease,PD)患者抑郁症状与睡眠障碍及认知功能的相关性。

方法 本研究采用回顾性分析

方法,分析我院就诊的PD患者的临床数据共计100例,选例时间为2022年1月至2024年1月。依据汉密尔顿抑郁-17(Hamilton depression -17,HAMD-17)评估抑郁症状,将PD伴抑郁症状患者归为抑郁组(HAMD-17≥7分,n=42),PD不伴抑郁症状患者归为非抑郁组(HAMD-17<7分,n=58)。同时按抑郁症状严重程度将PD伴抑郁症状患者进一步分为轻度抑郁组(n=24)、中度抑郁组(n=13)、重度抑郁组(n=5)。比较抑郁组和非抑郁组帕金森病睡眠量表-2(Parkinson's disease sleep scale-2,PDSS-2)评分、蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)评分,比较PD伴抑郁症状患者PDSS-2评分、MoCA评分,相关性分析PD患者抑郁症状与睡眠障碍及认知功能的关系。

结果 相比于非抑郁组,抑郁组PDSS-2评分更高,MoCA评分更低(P<0.05)。在PDSS-2评分上,重度抑郁组高于中度抑郁组,且中度抑郁组高于轻度抑郁组;在MoCA评分上,重度抑郁组低于中度抑郁组,且中度抑郁组低于轻度抑郁组(P<0.05)。相关性分析表明,PD患者HAMD-17评分与PDSS-2评分呈正相关,与MoCA评分呈负相关(P<0.05)。

结论 PD患者的抑郁症状与睡眠障碍及认知功能密切相关,抑郁症状越严重,睡眠障碍越突出,认知功能损害也越明显。

ABSTRACT

【Abstract】
Objective To analyze the correlation between depressive symptoms, sleep disorders and cognitive functions in patients with Parkinson's disease (PD).

Methods This study adopted a retrospective analysis method to analyze the clinical data of a total of 100 PD patients who visited our hospital. The case selection period was from January 2022 to January 2024. Depressive symptoms were evaluated based on Hamilton Depression-17 (HAMD-17). PD patients with depressive symptoms were classified as the depression group (HAMD-17≥7 points, n=42), and PD patients without depressive symptoms were classified as the non-depression group (HAMD-17 < 7 points, n=58). Meanwhile, patients with PD accompanied by depressive symptoms were further divided into the mild group (n=24), the moderate group (n=13), and the severe group (n=5) according to the severity of depressive symptoms. Compare the Parkinson's disease sleep scale-2 (PDSS-2) scores and the Montreal cognitive assessment (MoCA) scores between the depression group and the non-depression group. Compare the PDSS-2 score and MoCA score of patients with PD accompanied by depressive symptoms, and conduct a correlation analysis on the relationship between depressive symptoms and sleep disorders as well as cognitive function in PD patients.

Results Compared with the non-depression group, the PDSS-2 score of the depression group was higher and the MoCA score was lower (P<0.05). In terms of the PDSS-2 score, the severe depression group was higher than the moderate depression group, and the moderate depression group was higher than the mild depression group. In terms of MoCA scores, the severe depression group was lower than the moderate depression group, and the moderate depression group was lower than the mild depression group (P<0.05). Correlation analysis indicated that the HAMD-17 score of PD patients was positively correlated with the PDSS-2 score and negatively correlated with the MoCA score (P<0.05).

Conclusion   The depressive symptoms of PD patients are closely related to sleep disorders and cognitive function. The more severe the depressive symptoms, the more prominent the sleep disorders and the more obvious the cognitive function impairment.
